2011-12 Registration Materials

Fill out all required forms: (This packet was sent home the week of May 16, 2011)

•    Hot Lunch Form (optional)
•    Milk Sign-up Form (optional)
•    Gym Uniform Order for Grades 5-8
•    Bus Form
•    Extended Day Care Form (optional)
•    Room Parent form (optional)

Return all forms to the school office by Friday, May 27th.

Please do not send payment for any fees at this time.  A Statement will be sent for all tuition and fees later in the summer.  You will have the option of paying monthly or annually.

The class lists will be posted in the school lobby window on August 10th.

REGISTRATION INFORMATION:
Regardless of your choice of options, the following information is important:

•    The Hot Lunch Program is an optional program available on Wednesdays.  If you choose Hot Lunch, you must complete the lunch preference form.
•    Registration for our Extended Day Care program is enclosed with this packet.   Please complete these forms if you wish to use the EDP program for the 2011-2012 school year and return them to the office.
•    Gym Uniform Orders – please be sure to send in your order if you have a 5th through 8th grader.
•    Supply kits will be available for pick-up on August 11th.  If your kit is not picked up on that day, it will be placed in your child’s classroom.


VOLUNTEER SIGN-UP:
Volunteer sign-up will take place on the night of the Jr. High(Gr. 5-8) Open House, Wednesday, August 24th and the K-4 Open House, Thursday, September 1st.
